Bio

Dr. Jacqueline Klootwyk graduated from the University of the Sciences in Philadelphia, Philadelphia College of Pharmacy, in May 2006. After graduation, she completed an ambulatory care pharmacy residency, with an emphasis in teaching and education, at Midwestern University Chicago College of Pharmacy. After graduation from residency, she worked as an ambulatory care pharmacy practitioner at Shands Jacksonville in Jacksonville, Florida and as an assistant professor at Thomas Jefferson University, Jefferson School of Pharmacy in Philadelphia, PA. She became board certified in pharmacotherapy (BCPS) in 2008 and in ambulatory care (BCACP) in 2012. She continued her career in academia as an assistant professor at Duquesne University, Mylan School of Pharmacy in July 2013. She is interested in primary care, including anticoagulation, diabetes, dyslipidemia, and hypertension, as well as preventive and self-care.

Dr. Klootwyk is highly involved in the Pennsylvania Pharmacists Association (PPA). She currently sits on the Conference Committee where she helps organize the mid-year and annual meetings each year. She is also one of the OTC Student Competition Judges and has worked with the Health Policy Committee as well as the Transitions to Care Work Group within PPA. She contributed to the white paper entitle The Pharmacists' Role in the Patient-Centered Medical Home (PCMH) that was published in 2012 through the Health Policy Committee.
