Explain your theoretical orientation to counseling and counseling supervision. Identify your goals and explain why you intend to pursue a Ph.D. Provide a tentative outline of your research interests in counselor education and/or supervision. The maximum length of your Personal Statement should not exceed 750 words.

Email this form to two people willing to provide you with a reference. One (1) of the recommendation forms may be completed by a Duquesne counseling faculty member if the doctoral applicant is a graduate of Duquesne's masters' programs in counseling.

Email this form to one person who has been in a position to supervise your counseling and other professional activities.
