|CONSENT TO PARTICIPATE IN A RESEARCH STUDY|
|TITLE:||Visualization of Knowledge Sharing: Enhancing Online Motivation|

You are being asked to participate in a research project that seeks to investigate the impact of visualization of knowledge sharing activity on the motivation of individual to share knowledge to collaborative knowledge sharing spaces.
This study involves a series of online survey questions. You will be asked to respond to social capacity questions, participate in knowledge sharing activities, and answer questions regarding your willingness to continue sharing your knowledge. The survey should take about fifteen to thirty minutes to complete.
These are the only requests that will be made of you.
This study has been approved by Duquesne University Institutional Review Board.

|RISKS AND BENEFITS:||This project is not expected to involve any risks or harm beyond those of everyday life. No identifying information will be attached to any data collected. While there may be no direct benefit to you, your participation may contribute to the research that exists on knowledge sharing and motivation.|
|COMPENSATION:||There will be no monetary compensation to you, and participation will not cost you anything either. If you wish, however, you can enter a raffle to win an Apple iPad Air.|
No information will be collected or recorded which can be directly linked to you or your identity, and your ip address will not be recorded. Your identity will not be exposed in the data analysis. Your responses will only appear in statistical data summaries.
At the time of participation, SurveyMonkey, the online survey application, will assign an automated participation identification number (ID) for each participant. All of participants’ survey responses will be recorded in association with participation ID. Data transmitted to SurveyMonkey will be downloaded to the investigators’ password protected computers for analysis and will be securely maintained for a minimum of five years.
If you wish to enter the raffle, you will need to provide your email address at the end of the survey. The raffle sign-up page, however, is separate from the main survey, and there will be no way that your identity will be associated with your survey responses.
|RIGHT TO WITHDRAW:||You are under no obligation to participate in this study. Due to the anonymous nature of this study, the investigators cannot directly link you individually to any completed survey responses. If you wish to retract or discontinue your participation in the study, you may do so at any time. If you choose to withdraw during the study, any data already collected will not be used for the research. If you choose to withdraw after completion, however, due to the anonymous nature of the study, the data you provide will not be excluded from the final data analysis. Withdraw from the study will not result in any negative consequences for you.|
|SUMMARY OF RESULTS:||A summary of the results of this research will be supplied to you, at no cost, upon request.|
