Requesting Documents

We want to make the application process as easy as possible for you. To help you provide the necessary documents, here are some suggestions.

Requesting Official High School Transcripts

A written request is usually necessary to obtain official high school transcripts. If you attended more than one high school, contact the one from which you graduated. If your high school has closed or merged, mail your request to the local Board of Education or Diocesan Office. Your request should include: your name, address, birth date, social security number, name at the time of graduation if different, date of graduation, your signature, and date of request.

Requesting General Education Diploma (GED) Test Scores

A written request is necessary to obtain your GED test scores. Your request should include: your name, address, birth date, social security number, name at the time of test if different, location and date of test, your signature, and date of request. If your test was taken in Pennsylvania, mail your request to:

If your test was taken outside of Pennsylvania, contact the Board of Education in the appropriate school district. Degree applicants are to submit high school transcripts in addition to GED test scores. See instructions for requesting high school transcripts; indicate date of attendance rather than date of graduation on your request.

Requesting Official College Transcripts

Official transcripts should be requested from all colleges attended. A written request is necessary to obtain official college transcripts. Transcripts must be mailed directly to the School of Leadership and Professional Advancement by the issuing institution. Most colleges charge a fee. Contact individual institutions to determine transcript fees. Your request should include: your name, address, birth date, social security number, name at time of attendance or graduation if different, date of last attendance or graduation, your signature and date of request. Transcripts marked "Issued to Student" are not official. Visiting students are not required to submit transcripts.
