Health Forms for Students

New Students: Immunization and Health History Forms

Immunizations and health history documents are required for all students at Duquesne. 

All freshmen and other new students must submit these two forms before they will be allowed to register for classes. Please download, print, complete and mail or fax to the Health Service. (Fax number and address are on the forms.)

Pre-clinical Requirements

Complete this form only if you have been directed to do so by a school administrator. (This is not required for all new students.)

All Nursing students ( Freshman, transfer, 2nd degree, graduate), and all Pharmacy and Health Science students prior to their professional phase (generally 2nd to 3rd  year) need to meet the additional requirements as specified on this form.
