- Letter of Interest (A letter that accompanies your application that describes your background and education/career goals. Please limit to two pages. Email to slpaadmissions@duq.edu.)
- Official high school transcripts*. A GED diploma may be submitted in lieu of a high school transcript.
- Any official college and/or university transcript(s) with prior credits.*

- A minimum of two years professional experience
- A 500-word statement describing your academic and professional goals (email to slpaadmissions@duq.edu)
