International Student Health Requirements
Welcome to Duquesne University!
ALL INTERNATIONAL STUDENTS ENROLLED AT DUQUESNE UNIVERSITY ARE REQUIRED TO SUBMIT THROUGH THE DUQUESNE UNIVERSITY HEALTH SERVICE: On or before arrival on campus - A completed Health History , a completed Immunization (vaccine) RECORD AND translated (English) hard copy immunization records (A copy of YOUR childhood & adult immunizations- a record with dates of administration of vaccinations from your doctor or hospital)
IMMUNIZATION RECORD and HEALTH HISTORY (FOUND IN "TO DO" LIST IN STUDENT PORTAL)
All new students MUST submit the record of their immunizations and complete the health history through the Duquesne University Health Service electronic medical record system. Students have access to the Health Service Student Portal (LINK TO INSTRUCTIONS) through DORI using multipass log in, going to the Student Tab, and then clicking on "Student Connections." Through this portal, health history and immunizations can be submitted for easy verification by health service staff.
DUQUESNE UNIVERSITY REQUIREMENTS:
A COMPLETED HEALTH HISTORY
• Not a physical exam done by a doctor, only a list of questions about your health : SUBMIT THROUGH HEALTH SERVICE STUDENT PORTAL
EVIDENCE THAT YOU HAVE HAD TWO (2) MMR (Measles/Mumps/Rubella) IMMUNIZATIONS (VACCINATIONS)
• MMR Vaccine is given throughout the world to infants and young children to make them immune from those three childhood diseases. Becoming infected with them in adulthood is very dangerous.
• Please obtain these prior to your arrival. The cost for this vaccination is approximately US$100 if you must obtain the vaccine in the U.S.
TUBERCULOSIS TESTING (PPD), Q GOLD BLOOD TEST, OR NORMAL CHEST X-RAY
• Must be taken within the last two (2) weeks to assure that you are not infected with Tuberculosis, an extremely contagious bacterium that infects the lungs.
IF YOU PLAN TO LIVE ON CAMPUS IN ONE OF THE RESIDENCES, YOU ALSO MUST HAVE A:
MENINGITIS QUADRIVALENT VACCINE, or you may sign a waiver in person in the health office if you decline.
NOTE: ANY medical records brought from your home-country must be translated for the health care professionals in the U.S., and signed by your doctor.
IF THESE REQUIREMENTS ARE NOT COMPLETED PRIOR TO YOUR ARRIVAL, THEY MUST BE COMPLETED UPON ARRIVAL.
You must also submit a hard copy of your immunizations (vaccinations) by mail, fax, or in person with a Health Care Provider Signature on form or official documentation. (you may provide written proof of dates of immunization from one of the following: Official School health record, Physician record on letterhead, International Health Certificate, Official state certificate, Health Passport, Baby Book.) cited: Pennsylvania Department of Health State Immunization Code 23.82
Failure to comply with Health Services will affect your immigration. Students who do not meet with Health Services will have a hold on their registration. Students who can't register will violate their student status since they must be registered each semester to maintain F-1 status.
