International Student Health Requirements

Welcome to Duquesne University!

ALL INTERNATIONAL STUDENTS WHO ENROLL AT DUQUESNE UNIVERSITY ARE REQUIRED TO PROVIDE THE STUDENT HEALTH SERVICES DEPARTMENT WITH:

 A COMPLETED HEALTH HISTORY

• Not a physical exam done by a doctor, only a list of questions about your health

Health History Form

Please open and complete forms, print, and mail or fax to the Health Service. (Fax number and address are on the forms.)

Some personal computers have software programs that permit saving the completed documents. Completed & saved documents can be sent electronicaly to: duhealth@duq.edu

 

 EVIDENCE THAT YOU HAVE HAD TWO (2) MMR (Measles/Mumps/Rubella) IMMUNIZATIONS

Immunization Record Form

• MMR Vaccine is given throughout the world to infants and young children to make them immune from those three childhood diseases. Becoming infected with them in adulthood is very dangerous.
• Please obtain these prior to your arrival. The cost for this vaccination is approximately US$100 if you must obtain the vaccine in the U.S. 

 

 TUBERCULOSIS TESTING (PPD) OR NORMAL CHEST X-RAY

 • Must be taken within the last two (2) weeks to assure that you are not infected with Tuberculosis, an extremely contagious bacterium that infects the lungs.

 

    IF YOU PLAN LIVE ON CAMPUS IN ONE OF THE RESIDENCES, YOU ALSO MUST HAVE A:
 MENINGITIS VACCINE, or you may sign a waiver if you decline.

 

NOTE: ANY medical records brought from your home-country must be translated for the health care professionals in the U.S., and signed by your doctor.
IF THESE REQUIREMENTS ARE NOT COMPLETED PRIOR TO YOUR ARRIVAL, THEY MUST BE COMPLETED UPON ARRIVAL.

Failure to comply with Health Services will affect your immigration.  Students who do not meet with Health Services will have a hold on their registration.  Students who can't register will violate their student status since they must be registered each semester to maintain F-1 status.
