Cigna Dental PPO - Core Network
Cigna offers two dental plan options – Basic and Enhanced
- Both plans offer the freedom of receiving services from the dentist of your choice.
- Payment for covered services is based upon a percentage of the usual, customary, and reasonable (UCR) amounts.
- Participating dentists receive payment directly from Cigna and agree to accept the UCR allowances as payment in full (less applicable deductible and coinsurance) and will also file claims on your behalf.
- Cigna Dental does not issue dental ID cards. Cigna medical participants can use their medical ID cards to obtain dental coverage as their ID information is the same. Dental particiants that do not use Cigna as their medical provider can use a universal ID card along with the employee's Social Security Number to obtain dental services.
The plan will pay up to the UCR allowance to non-participating dentists, but they are under no obligation to accept this allowance as payment in full. Any excess amount will be your responsibility. You may also be responsible for filing claims when you use a non-participating dentist. Payment will be made to you, and you will be responsible for paying the dentist.
Predetermine Your Eligibility
Predetermination is recommended, but not required, by Cigna to determine your eligibility and the extent of coverage for extensive treatment plans involving prosthetics, crowns, inlays, onlays and periodontics. This process assures both you and your dentist that the service being performed is covered and provides you with your financial liability.
