  • Loss of life, both hands, feet, eyes 100% of coverage level (in addition to life insurance benefit for loss of life)
  • Loss of one hand, one foot, one eye 50% of coverage level

Additional 1/2, 1, 2, 3, 4, or 5 times salary
Maximum $750,000
MEI Medical Evidence of Insurability
Required if amount is over the lesser of 3 times salary or $300,000 or increasing more than one level during Open Enrollment.
Imputed Income IRS requires that the cost of employer sponsored term insurance in excess of $50,000 be treated as imputed income. This amount is shown on your pay stub and will be reported to the IRS on your W-2 form. Long Term Disability

Long Term Disability replaces a portion of your income if illness or accident prevents you from working for an extended period of time.

Maximum of $5,000 per month
Premium is paid by Duquesne University
Buy Up Option  60% of base salary
Maximum of $10,000 per month

Employee is responsible for applying for LTD benefits. LTD claim forms are available from the Disability Claims Manager.
