Program Forms

Students accepted into the Master of Science in Educational Studies Program will find the following forms useful:

  • Advisement Sheet
    A replica of the official advisement sheet emailed to students each semester prior to registration.

  • Application for Degree
    Complete at the start of your last semester before graduation. Students who do not submit this paperwork will not be permitted to graduate. Consult deadlines on the Academic Calendar or contact the Program Office for assistance.

  • Catholic Lay Teacher/Administrator Discount
    Students who teach in Catholic Schools in Pittsburgh, Greensburg or Altoona-Johnstown Diocese may qualify for a lay teacher discount. Contact the Director of Financial Aid for details and criteria: 412-396-6607

  • Change of Schedule
    Use this form to drop or add a course.
