Verification of Enrollment
A verification of enrollment is written proof that you are or were an enrolled student at Duquesne University. Verification is often requested for a job, loan or insurance.
How to Request a Verification of Enrollment
Current students can log onto DORI and access Self-Service Banner to request an Enrollment Verification form.

Verification of Enrollment Form
Former students can download, sign and return the Enrollment Verification Form.
Requests cannot be accepted over the phone, email or by a third party. Processing this information will take one to two business days.
Please provide the following information:
- Your full name
- Last four digits of your social security number
- What years of attendance you want to verify
- Where you want the information to be sent
- Signature
