MetLife PDP Plus Network
Your dental benefits are provided through MetLife Preferred Dentist Provider (PDP) plan which maintains a network of dental providers across the country.
MetLife Preferred Dentist Provider (PDP) plan does not provide identification cards. In-network providers automatically submit electronic claims on your behalf.
TWO PLANS. The MetLife (PDP) plan provides two options for coverage - Basic and Enhanced. Both plans offer the freedom to choose any provider, however, use of an in-network provider offers additional benefits. Review the benefit grid to determine the plan that best suits your needs.
OBTAINING SERVICES OUT-OF-NETWORK. If you choose a nonparticipating dentist, your out-of-pocket costs may be higher. A non-participating dentist has not agreed to accept negotiated fees. So you may be responsible for any difference in cost between the dentist's fee and your plan's benefit payment. The dental provider may require payment in full at the time services are rendered. Submit claims to MetLife with itemized receipts, along with full patient and member information to MetLife Dental Claims, P.O. Box 981282, El Paso, TX 79998-1282 or fax to 859.389.6505.
PRE-TREATMENT ESTIMATE. If your dental care will be extensive, you may ask your dentist to complete and submit a request for cost estimate, sometimes called a pre-treatment or pre-determination of benefits. This will allow you to know in advance what procedures are covered, the amount the benefit plan will pay toward treatment and your financial responsibility.
FLEXIBLE SPENDING ACCOUNTS. MetLife dental has an established relationship with Discovery Benefits, our Flexible Spending Account provider. MetLife dental automatically sends a monthly report to Discovery Benefits to establish documentation.

- Visiting your dentist regularly for checkups
- Following your dentist's advice about regular brushing and flossing
- Seeking treatment before you have a major problem
