Federal Verification Forms
2019-2020 Academic Year
Students can access their required Federal Verification forms and other requirements online in Self Service Banner through the DORI portal.
Instructions for accessing Financial Aid required documents/forms through Self Service Banner:
- New and Prospective Students: Set up your MultiPass by visiting www.duq.edu/multipass
- Click Setup MultiPass
- Follow the on-screen prompts & enter the required criteria
- Call the Help Desk if you have questions/problems at 412-396-HELP (4357)
- Log in to DORI: www.duq.edu/dori using your MultiPass and password
- New and Prospective Students: Click the Financial Aid Self Service link located on the left column
- Current Students: Click the Self Service Banner link
- Click the Financial Aid tab
- Click Required Documents/Forms
- Select Aid Year: Academic Year 2019-2020, click Submit
- Review your requirements. Instructions and links are listed for each requirement. Contact the Financial Aid Office with any questions at 412-396-6607.
Secure Document Upload: For your convenience, electronic documents can be securely submitted using our Secure Document Upload application.
2018-2019 Academic Year
The forms for the 2018-2019 year are below. Select the forms for the academic year that we have requested. Please scroll down to access the forms.
IMPORTANT: Please only complete the following requirements if specifically requested by the Financial Aid Office.
For your convenience, electronic documents can be securely submitted using our Secure Document Upload application.
|2016 IRS Tax Return||
If you have not done so, you can complete this requirement by using the IRS Data Retrieval Tool (IRS DRT) which is a part of FAFSA on the Web at https://studentaid.ed.gov/sa/fafsa.
Otherwise, please submit a signed copy of the 2016 Federal Income Tax Return.
If the Federal Income Tax return is not available, please submit an IRS Tax Return Transcript. Go to www.irs.gov/Individuals/Get-Transcript and complete the request process. Make sure to request the "Return Transcript" and NOT the "Account Transcript". Only submit if requested.
|Household Information Form||Please complete and submit the Federal Verification Household Information form for 2018-2019 only if requested.|
|2016 W-2 Forms||Please submit a copy of W-2 Form(s) only if requested|
|IRS Verification of Non-filing Letter||
The Verification of Non-filing Letter from the IRS provides proof that the IRS has no record of a filed Form 1040, 1040A or 1040EZ for the year requested. This letter is available after June 15 for the current tax year or anytime for the prior three tax years using Get Transcript Online. Please go to https://www.irs.gov/individuals/get-transcript and click the "Get Transcript Online" button to start the request process.
If you encounter any issues with obtaining this letter from the IRS, please contact the Financial Aid Office for assistance.
|Amended Tax Return 2016 IRS Form 1040X||Please provide a signed copy of your amended 2016 federal tax return - Form 1040X. Only required if specifically requested.|
|Federal Reject, review your SAR for the resolution needed||If you have received this notice, your financial aid cannot be processed due to a Federal Student Aid Report (SAR) reject issue. Please contact our office for help and additional information if necessary.|
|FAFSA issue to resolve: Selective Service||The Selective Service reported that you have not registered with them. If you are female or were born before 1960, registration is not required. Otherwise, if you are not yet registered, are male, and are 18 through 25 years of age, to receive aid you must do one of the following: 1) Register online at www.sss.gov, or 2) Request registration on your Student Aid report by answering "Male" to item 21 and "Register Me" to item 22, or 3) Complete a Selective Service registration form at your local post office. If you have documentation proving an exemption, submit the documentation to the Financial Aid Office.|
|FAFSA issue to resolve...||If you receive a notice of this type, please contact the Financial Aid Office for assistance in resolving the FAFSA issue.|
|2018-2019 Duquesne Financial Aid Application||
Please complete the Duquesne University Financial Aid Application if requested. This application is only required if you plan to attend part-time or if you will be a graduate student.
**If you will be/are a full-time undergraduate student, this application IS NOT required.
|Full name mismatch; documentation required||
We have conflicting information for the student's first or last name. Your name in our database does not match the name used on the FAFSA. Please provide documentation of your legal name such as a copy of your drivers license, a copy of your birth certificate, etc.
