PA Acts 33 & 34 Clearances

You may be required to apply for your Act 33 and Act 34 clearances when working with children or volunteering with vulnerable populations.

Act 33 Child Abuse Form

Once you are on the form website, search for Child Abuse Form, scroll down and click on Child Abuse History Clearance Forms. Then complete the follow steps:

  1. Print out the form. Complete Section 1 in its entirety using BLUE or BLACK ink. Please print legibly or your form may be returned unprocessed.
  2. Under "Applicant Identification" clearly print your name and CURRENT address (your school address, or where you commute to Duquesne from if you do not live on campus, not your permanent or parents' address). The address you put down will be where results are sent.
  3. Under "Purpose of Clearance" please check "School".
  4. Complete the sections regarding previous addresses and household members and sign and date the application. These sections must be completed in their entirety or your form may be returned unprocessed.
  5. Send the completed application along with a $10.00 money order (do not send cash or a personal check) made payable to the Department of Public Welfare to:

You will receive your clearance via mail at the address you listed under "Applicant Identification." This process will take approximately 14 business days.

We suggest making several copies of your clearance as you may be required to have them for various circumstances in the futures.

Act 34 Criminal Record Check Form

This form can be completed online or by mail.

To complete the form online and pay with a credit card:

  1. On the PA State Police web page, use the PATCH System to process your check.
  2. Underneath "Welcome to Pennsylvania Access To Criminal History," click "Submit a New Record Check" under the heading "Credit Card Users."
  3. Review the "Terms and Conditions for the use of PATCH." Scroll to the bottom of the page. To begin the process, click "Accept." If you click "Decline" you cannot obtain your clearances online.
  4. Move through the remaining pages as directed to pay for and receive your clearance printout. (A credit card is required to pay for each clearance request.)

Unless there is a need for extensive research by the PSP, you will receive a printable version of your clearance after the application has been completed online. WE SUGGEST MAKING SEVERAL COPIES OF THE RESULTS AS YOU MAY BE REQUIRED TO HAVE THEM FOR VARIOUS CIRCUMSTANCES IN THE FUTURE.


To mail a copy and pay by money order please click here.

  1. On the PA State Police web page, click on "Download the Criminal History Request Form - SP4-164" and print the form.
  2. Clearly print your name as the ‘Requester Name' and include your CURRENT address (your school address, or where you commute to Duquesne from if you do not live on campus, not your permanent or parents' address). The address you put down will be where results are sent.
  3. Under "Reason for Request" check "Employment" for work study; for all other requests check "Other" and specify "Volunteer" next to it.
  4. Send the completed application along with a money order (do not send cash or a personal check) made payable to Commonwealth of Pennsylvania to:

You will receive your clearance by mail at the address you listed under "Requester name." This process will take approximately 14 business days.

How to Obtain a Money Order:
Money orders can be purchased with cash, debit cards or travelers checks at US post offices, most banks, and many grocery store customer service desks, convenience stores, and check-cashing businesses.

Be prepared to pay anywhere from 50 cents to a $4.00 fee for the money order.
